The healthcare crisis in the U.S. is one marked by rising costs, coverage gaps, staggering medical debt, and attacks on access. While various groups have stepped up with innovative solutions to address these serious issues, experts say the crisis is likely to get worse in the absence of radical policy change at the federal level. Consider how Undue Medical Debt is tackling the $220 billion in medical debt that affects some 100 million Americans. Since the nonprofit was founded more than a decade ago, it has forgiven $27 billion in debt for 17 million people by buying debt for pennies on the dollar using donations. While states have stepped in with some measures to protect consumers from more serious financial ramifications that might stem from their medical debt, such actions—and the work that Undue Medical Debt is doing—are merely a Band-Aid of sorts. “It is a systems problem and we have to address it in a systems way, and the system is very much broken for people,” Sesso said. OTHER SOLUTIONS One of those problems is that there’s a ripple effect to federal policy changes, like the approximately $1 trillion that will be cut from the Medicaid program over the next decade. That will increase costs for everyone, which penalizes employers and employees who rely on employer-sponsored healthcare plans, noted Shelly Towns, chief marketing officer at Lantern, a company that helps bring down costs related to specialty care, including cancer and surgery. “We are trying to steer as many members as possible into low-cost care, which can come with a very high-quality bar—which is what we require,” Towns said. “And you can just save a ton of money by doing that.” Finally, another route for addressing the current crisis is through the courts. Of the 200-plus legal actions that the American Civil Liberties Union (ACLU) has filed, it has won 64% of them, said Ambalika Williams, senior campaign strategist for reproductive freedom at the civil rights organization. What happens to your student loans now that the SAVE plan is dead?
After a long court battle, the SAVE plan is officially kaput. Launched in 2023, the Biden administration’s Saving on a Valuable Education (SAVE) federal student loan repayment plan was created to replace the outgoing REPAYE program–and help keep Biden’s campaign promise to forgive student loans. Under the SAVE plan, a borrower’s monthly payment would be calculated based on income and family size and could be set as low as $0 per month for the lowest-earning borrowers. The program also fast-tracked forgiveness for those who borrowed less than $12,000. Several states sued the Biden administration in 2024, arguing that the SAVE plan exceeded the administrative branch’s statutory authority. Payments were paused for borrowers enrolled in the SAVE program during the litigation process, but interest began accruing on the loans as of August 2025. The Eighth Circuit Court of Appeals decision on March 10 officially ended the SAVE plan for good–and borrowers are left with the less-than-reassuring message that they can expect “clear guidance” from the Department of Education in the coming weeks. To get some direct answers about what student loan borrowers should do about the end of the SAVE program, I talked to student loan expert and author Mark Kantrowitz. Here’s what you need to know. Stick a fork in it—SAVE is done Some of the reporting about the end of the SAVE plan has included information about the Havens v. U.S. Department of Education (ED) lawsuit which was filed hours before the Eighth Circuit Court’s decision. This lawsuit argues that the ED is compelled to implement the SAVE plan. But Kantrowitz makes it clear there is no hail mary that can save the SAVE plan. “The Havens v. U.S. Department of Education lawsuit was rendered moot by the Eighth Circuit Court’s decision ending the SAVE plan,” he says. “It will not resurrect the SAVE repayment plan or provide the forgiveness sought by the plaintiffs.” Available repayment options Current borrowers on the SAVE plan have the following repayment plan options. Borrowers can access these options by filling out the Income Driven Repayment (IDR) Plan Request Form. Income-Based Repayment (IBR) This repayment plan sets your monthly payment as 10% or 15% of your discretionary income, depending on when your loans were originally disbursed. After 20 or 25 years of repayment, any remaining loan balance is forgiven. Pay As You Earn (PAYE) & Income-Contingent Repayment (ICR) Under the PAYE plan, your monthly payments are capped at 10% of your discretionary income. After 20 years of repayment, any remaining loan balance is forgiven. The ICR plan calculates your monthly payment as either 20% of your discretionary income or the amount you would pay on a fixed 12-year plan, whichever is less. After 25 years of repayment, any remaining loan balance is forgiven. Both the PAYE and ICR plans are currently available to SAVE borrowers, but they are scheduled to be phased out in July, 2028. If you choose to switch to PAYE or ICR today, you will need to choose a different repayment plan as of July 2028. Repayment Assistance Program (RAP)—Coming July 2026 Under this forthcoming repayment plan, monthly payments will range from 1% to 10% of your adjusted gross income, with a minimum monthly payment of $10. As long as you are making payments, RAP will cancel any unpaid interest so your balance can’t continue to grow. After 30 years of repayment, any remaining loan balance is forgiven. Although SAVE borrowers cannot sign up for this repayment plan currently, it will be available this summer. No need to wait for ED guidance The end of the SAVE plan instantly put the seven million borrowers who are currently signed up for the program into a strange sort of limbo. These borrowers have not had to make payments on their loans since litigation began in 2024, although interest has been accruing on their loans for the past seven months. But when the Eighth Circuit Court decision struck down the SAVE plan, the ED was unable to offer clear guidance about when or how these borrowers would have to switch their repayment plans. Kantrowitz explains that this guidance is probably going to take a little longer to materialize, since the Eighth Circuit Court decision reversed a lower court’s decision. That doesn’t change the fact that SAVE is dead, but it does mean there’s still a little more legal volleying. “Once the lower court accepts the settlement between Missouri and the U.S. Department of Education, the SAVE plan will end,” Kantrowitz says. “Then the U.S. Department of Education will provide guidance about switching into another repayment plan.” Remember that your interest is accruing The seven million borrowers scrambling to change their repayment plan are unfortunately being met with a Department of Education that didn’t prepare for them. “The IDR Plan Request Form has a backlog of 576,609 applications left to process, which will take an additional seven months at the current rate,” Kantrowitz says. “If there are no improvements and the 7.2 million SAVE borrowers file this form, it will take an additional seven years to clear the backlog.” If waiting for the ED to deal with this doesn’t sound enough like a Kafka-esque nightmare, Kantrowitz points out that the delay will also stab you right in the bank account. “Interest will continue to accrue on these loans while the borrowers are waiting to switch into IBR,” he says. The SAVE plan is dead, so what does that mean for your student?
After a long court battle, the SAVE plan is officially kaput. Launched in 2023, the Biden administration’s Saving on a Valuable Education (SAVE) federal student loan repayment plan was created to replace the outgoing REPAYE program–and help keep Biden’s campaign promise to forgive student loans. Under the SAVE plan, a borrower’s monthly payment would be calculated based on income and family size and could be set as low as $0 per month for the lowest-earning borrowers. The program also fast-tracked forgiveness for those who borrowed less than $12,000. Several states sued the Biden administration in 2024, arguing that the SAVE plan exceeded the administrative branch’s statutory authority. Payments were paused for borrowers enrolled in the SAVE program during the litigation process, but interest began accruing on the loans as of August 2025. The Eighth Circuit Court of Appeals decision on March 10 officially ended the SAVE plan for good–and borrowers are left with the less-than-reassuring message that they can expect “clear guidance” from the Department of Education in the coming weeks. To get some direct answers about what student loan borrowers should do about the end of the SAVE program, I talked to student loan expert and author Mark Kantrowitz. Here’s what you need to know. Stick a fork in it – SAVE is done Some of the reporting about the end of the SAVE plan has included information about the Havens v. U.S. Department of Education (ED) lawsuit which was filed hours before the Eighth Circuit Court’s decision. This lawsuit argues that the ED is compelled to implement the SAVE plan. But Kantrowitz makes it clear there is no hail mary that can save the SAVE plan. “The Havens v. U.S. Department of Education lawsuit was rendered moot by the Eighth Circuit Court’s decision ending the SAVE plan,” he says. “It will not resurrect the SAVE repayment plan or provide the forgiveness sought by the plaintiffs.” Available repayment options Current borrowers on the SAVE plan have the following repayment plan options. Borrowers can access these options by filling out the Income Driven Repayment (IDR) Plan Request Form. Income-Based Repayment (IBR) This repayment plan sets your monthly payment as 10% or 15% of your discretionary income, depending on when your loans were originally disbursed. After 20 or 25 years of repayment, any remaining loan balance is forgiven. Pay As You Earn (PAYE) & Income-Contingent Repayment (ICR) Under the PAYE plan, your monthly payments are capped at 10% of your discretionary income. After 20 years of repayment, any remaining loan balance is forgiven. The ICR plan calculates your monthly payment as either 20% of your discretionary income or the amount you would pay on a fixed 12-year plan, whichever is less. After 25 years of repayment, any remaining loan balance is forgiven. Both the PAYE and ICR plans are currently available to SAVE borrowers, but they are scheduled to be phased out in July, 2028. If you choose to switch to PAYE or ICR today, you will need to choose a different repayment plan as of July 2028. Repayment Assistance Program (RAP) – Coming July 2026 Under this forthcoming repayment plan, monthly payments will range from 1% to 10% of your adjusted gross income, with a minimum monthly payment of $10. As long as you are making payments, RAP will cancel any unpaid interest so your balance can’t continue to grow. After 30 years of repayment, any remaining loan balance is forgiven. Although SAVE borrowers cannot sign up for this repayment plan currently, it will be available this summer. No need to wait for ED guidance The end of the SAVE plan instantly put the 7 million borrowers who are currently signed up for the program into a strange sort of limbo. These borrowers have not had to make payments on their loans since litigation began in 2024, although interest has been accruing on their loans for the past seven months. But when the Eighth Circuit Court decision struck down the SAVE plan, the ED was unable to offer clear guidance about when or how these borrowers would have to switch their repayment plans. Kantrowitz explains that this guidance is probably going to take a little longer to materialize, since the Eighth Circuit Court decision reversed a lower court’s decision. That doesn’t change the fact that SAVE is dead, but it does mean there’s still a little more legal volleying. “Once the lower court accepts the settlement between Missouri and the U.S. Department of Education, the SAVE plan will end,” Kantrowitz says. “Then the U.S. Department of Education will provide guidance about switching into another repayment plan.” Remember that your interest is accruing The 7 million borrowers scrambling to change their repayment plan are unfortunately being met with a Department of Education that didn’t prepare for them. “The IDR Plan Request Form has a backlog of 576,609 applications left to process, which will take an additional 7 months at the current rate,” Kantrowitz says. “If there are no improvements and the 7.2 million SAVE borrowers file this form, it will take an additional 7 years to clear the backlog.” If waiting for the ED to deal with this doesn’t sound enough like a Kafka-esque nightmare, Kantrowitz points out that the delay will also stab you right in the bank account. “Interest will continue to accrue on these loans while the borrowers are waiting to switch into IBR,” he says. If you don’t trust Big Tech with your data, this is the chatbot for you
Sure, when chatbots aren’t outright hallucinating, they can be helpful tools for gathering information, generating ideas, and completing tasks. But some of the biggest players in the AI chatbot space—including OpenAI, Google, and Meta—aren’t exactly known for strong privacy protections. So you have to have a lot of blind faith that the data you give to their chatbots won’t be used in ways you might not like, such as building a profile around you and your prompt history for the purposes of advertising or tracking. So, what’s a person to do if they don’t trust Big Tech with their chatbot data? Give up AI chatbots entirely? Luckily, they don’t have to. I’ve been testing out a relatively small player in the AI chatbot market for a few weeks now called Duck.ai. It offers access to some of the most powerful LLMs out there while protecting my privacy from them. Now, I can’t imagine myself ever going back to using one of the behemoths directly. ChatGPT without the privacy fears DuckDuckGo is best known for its privacy-focused search engine. Unlike Google, DuckDuckGo doesn’t track your search history or create a profile of your online activity. This privacy-first approach is one reason Apple, another company that values user privacy, allows people to set DuckDuckGo as the default search engine in its Safari browser—and it’s the main reason many choose DuckDuckGo, which was founded in 2008, over Google. A few years ago, DuckDuckGo entered the AI chatbot market with the launch of Duck.ai. This chatbot differs from most major ones, such as ChatGPT from OpenAI or Gemini from Google, in two key ways. The first is that Duck.ai doesn’t have its own “DuckDuckGo” LLM. While its interface is nearly identical to most chatbot interfaces—with a prompt box in the center and a sidebar on the left showing your chat history—Duck.ai features a drop-down menu that lets you choose among various LLM models. You can pick from six free models for each new prompt, with five additional models available if you subscribe to DuckDuckGo’s VPN service, which costs $9.99 per month or $99 per year. In other words, Duck.ai is an LLM aggregator. You use a single interface, but with each new prompt you can choose a different LLM to respond. Free options include OpenAI’s ChatGPT GPT-4o mini and GPT-5 mini, Meta’s Llama 4 Scout, and Anthropic’s Claude Haiku 4.5, among others. Subscribers gain access to additional models, including OpenAI’s ChatGPT GPT-4o, GPT-5.2, and GPT-5.2 Thinking, as well as Anthropic’s Claude Sonnet 4.5 and Meta’s Llama 4 Maverick. The convenience of not having to switch among different websites—and of keeping your entire conversation history across multiple LLMs in one place—is enough reason to love Duck.ai. But as you might have surmised by now, the main advantage of the Duck.ai chatbot is privacy. If you’ve ever wished you could use Meta’s or OpenAI’s LLMs without those companies tracking you—well, now, you can. Duck.ai keeps your prompts private Even though Duck.ai allows you to use various versions of OpenAI’s ChatGPT models, OpenAI will never know that you, specifically, are the one typing the prompts (provided you don’t directly or indirectly identify yourself through the prompts themselves). That’s because no matter which model you select in Duck.ai, DuckDuckGo strips all metadata from your query before sending your prompt to the LLM provider for processing. Critically, this metadata includes your IP address, which is one of the main ways Big Tech companies know when the same person is using their services. But Duck.ai goes further in protecting your privacy. In addition to stripping metadata from your prompts, DuckDuckGo will not use your queries to train any of its AI, and DuckDuckGo also has agreements in place with the model providers it works with to ensure that none of your prompts or associated data is used to train their AI models. Those agreements also state that the model providers must delete your anonymized prompts within 30 days (subject to legal and safety compliance rules). But there’s a third huge advantage of using Duck.ai. For most LLMs, such as OpenAI’s ChatGPT and Google’s Gemini, if you want the chatbot to save your chat history so that you can reference your chats in the future, you need to create an account. But by creating that account, the chatbot provider knows who you are. Duck.ai allows even anonymous users to store their chat history. It can do this because Duck.ai chats are stored locally on the user’s device (their laptop or phone) and not on Duck.ai’s servers. This means that while Duck.ai and its LLM providers will never keep a record of your chats, you can still maintain one yourself (provided you don’t clear your browser’s cache). Of course, users can also choose to disable local saved history on their devices so that others who use the same computer can’t see their prompting history. And Duck.ai even provides a one-click “burn” button to quickly wipe a chat from your computer. So, what are the drawbacks? After using Duck.ai for a few weeks, it’s become my go-to chatbot. However, there are some important caveats I should add to that statement. First, I don’t consider myself a “heavy” chatbot user. I mostly use it for quick searches or light research (and almost never trust the results without fact-checking them with external sources). My relatively light chatbot usage means I don’t require the latest and greatest AI models. If you do, then Duck.ai might not be for you. That’s because the “free” models every user of Duck.ai has access to are old by LLM standards (for example, OpenAI’s GPT-4o and GPT-4o mini came out nearly two years ago). Those who subscribe to DuckDuckGo’s VPN service (which also offers identity theft restoration services) have access to newer models, including OpenAI’s GPT-5.2 /Thinking. However, even that model is several months old and has been surpassed by GPT-5.3 and GPT-5.4. A second caveat is that while Duck.ai offers LLMs from OpenAI, Anthropic, and Meta, among others, it doesn’t bring together all the most popular chatbots in one place. For example, Perplexity, DeepSeek, and Google’s Gemini are not options in Duck.ai. If you use those LLMs, Duck.ai is not currently suitable for you. Research from the University of Washington confirms this “early riser bias”: employees who start early are rated as more conscientious and receive higher performance evaluations, even when they work the same hours as colleagues who start later. It sounds like an advantage—and it is. But for many early chronotypes, that same structure becomes a trap. Because the day is already tilted in your favor, it’s easy to slide into overwork and under-recovery. I’ve spent more than 20 years studying how biological rhythms shape performance and well-being, and here’s what I’ve learned: success for morning types isn’t about working more. It’s about working with your rhythm consciously—so your energy, creativity, and relationships can last beyond the morning peak. Here are three strategies that can help. 1. Protect your energy for the most complex tasks Your brain’s peak performance window is in the early hours. That’s your zone for focus, complex problem-solving, and creative thinking—but only if you protect it. Carve out dedicated time in your schedule to complete the most important projects first (and make sure you get early morning daylight outside, which helps keep your biological rhythms in sync). Don’t let those hours disappear into emails, calls, or meetings that could happen later. One of my clients, Bettina, naturally wakes around 4 a.m. and uses those quiet early hours for her most complex work—strategy, analysis, creative thinking—before the digital noise and meetings begin. By noon, she’s already delivered her day’s most important results. If your workplace isn’t flexible, carve out even one “no meeting” morning a week. Let your team know you’re focusing on high-value work during those hours. Most managers appreciate the clarity when it’s framed around results, not routines. 2. Schedule your rest Morning types are good at starting early—but not always at stopping. Because your rhythm peaks early, it also winds down earlier. If you keep working into the evening or saying yes to late-night events, you’re running against your biology. Aim to finish work by late afternoon. Dim the lights and screens early, and go to bed at a consistent time—even 8:30 or 9 p.m. if that’s when your body asks for it. How you close the day shapes how you begin the next. Quality rest doesn’t just recharge you—it stabilizes your circadian rhythm, strengthens memory, and protects your emotional balance. When you stop glorifying being “always on,” you extend your peak performance window across the week. 3. Design your social life around your biology For many early risers, social life can feel like a punishment, not a pleasure. Dinner invitations arrive for 8 p.m.—exactly when your body wants to wind down. There’s often subtle social judgment: “You’re leaving already?” or “Must be nice to need so much sleep.” But here’s the truth: you don’t need more sleep than night owls—you need it at a different time. And trying to force yourself into evening social patterns doesn’t make you more social. It makes you exhausted, irritable, and less present. Instead, reshape connection around daylight. You and I talked in what was the early phase of GLP-1 mania as weight-loss drugs Ozempic, Wegovy, and Zepbound were becoming household names. Since then, you have been in the heart of the action. I saw you had a Super Bowl ad this year featuring Serena Williams. I see your subway ads here in New York City with Serena and Charles Barkley all the time. Yeah, love that. I love them. I assume that having Reddit founder Alexis Ohanian—Serena’s husband—as an investor and on your board, that sort of helped make that possible, helped connect you to Serena? Definitely helped with the initial introduction, but I think we would give her all the credit in the world for telling the story and making it happen. A Super Bowl ad is expensive. What made you decide to do that? Because early on, it almost seemed like advertising wasn’t even a good idea with GLP-1s because there was enough supply. It’s a lot of money, but I actually don’t think it’s expensive when you think about it. So the key distinction there, it is the only time in the entire year when a hundred-million-plus people gather, and ads are part of the product. So all other times, they’re an interruption in your life. You’re trying to get past them. I mean, I genuinely enjoy ads and the storytelling behind them. My wife makes fun of me for it. But for the most time, when you’re watching a TV show or a football game, it’s the time where you pick up your phone or you go to the restroom. But during the Super Bowl, it’s actually the time where many people, not everyone, but many people pay the most attention. So it provides a very, very unique opportunity. For us, we announced our partnership with Serena in August, really trying to emphasize the concept that GLP-1s are not a shortcut. Weight management and weight loss and that journey, people who struggle with it, it’s often perceived as a lack of self-discipline or willpower, and that using a medication for it could be perceived as a shortcut. Well, I think it’s fairly obvious that there’s no one probably on the planet with more self-discipline and willpower than Serena Williams. That was the first myth we wanted to bust. We felt like she was the perfect person who uses every tool to optimize performance and optimize her health, but would never take a shortcut. She also talked about her improvement in blood sugar levels. She also talked about her reduction of heart disease, reduction of cholesterol by 30%, and her improvement in knee pain. But that was not the story that got picked up. So with the Super Bowl campaign, the intent was to try and break this second myth that GLP-1s are only for weight loss. The last time we spoke, I believe that GLP-1s were only approved for diabetes and newly obesity. That’s right. And the cash-pay prices were about $1,300. Fast-forward to today, they’re approved for diabetes, obesity, heart disease, kidney disease, liver disease, sleep apnea, being studied for neurodegenerative diseases, substance use disorders, pain management. So we felt, again, a very, very powerful story to tell. And it was not about needing more demand. I think it was about continuing to reach and give other people permission to seek out the best tools for their health. We talked about some of the backlash that was going on about GLP-1 ads when some folks were saying, “Oh, you’re kind of labeling folks who are carrying more weight as needing to change their bodies.” Did that give you any pause about the Super Bowl ad or any of the rest of this? Or has the cultural conversation moved away from that? I think there has been, fortunately, a reduction in the stigma, but it still very much exists, and it exists in all aspects of life. It exists when someone walks in the doctor’s office, it exists in social media, it exists with friends and family. It’s so wrapped up in so many different components of people’s lives. What is interesting is what was seen as shocking maybe three and a half, four years ago, our ad that we originally talked about was literally a subway campaign where it showed the medication, it showed the pen, and then it showed someone injecting the pen, and then it said “A weekly shot to lose weight,” and had the safety information. Yeah, it was a very calm, measured ad compared to the reaction, which was super emotional. It just said what the thing did, and it showed people using it as their doctor would direct them to. So I think that—a weekly shot to lose weight—probably is not surprising to people today. So we would say, “Hey, if we ran that same campaign, would people have that same reaction?” I think no. The fact that they wouldn’t have that same reaction is to me a sign that there has been some level of improvement. I think we’ll look back five years from now that Serena sharing her story is sort of this watershed moment. When Hims & Hers announced a $49-a-month version of the Wegovy pill earlier this year, the Food and Drug Administration stepped in to shut it down. What’s the competitive landscape there? Do you look at Hims & Hers as sort of a brother in arms in building this space? Or is it a nemesis that makes things more difficult for you? It’s very similar to, I would say, like driving a car: You have a destination in mind, that destination is not identical to everyone else. But 5% of the time when you’re driving the car, you’re looking left, you’re looking right, you’re looking behind you, you’re aware of your surroundings. I’d say we look at competition very similarly. Ro is the leading provider of branded GLP-1s in the country. Absolute No. 1, far and away. The reason that is the case is because we, I think, relative to all competitors—not naming one specifically, but all competitors—really work backwards from prioritizing what is absolutely best for patients. We offer cash-pay and insurance options. We have the strongest formulary. And then we have the highest-quality care, which we’ve published our data showing that patients on Ro lose the same amount of weight as they do in clinical trials, which is very rare, [to] see real-world evidence match clinical trials.
